Help with financial aid appeal letter?

i'm writing letter university's financial aid office. missed deadline loan, , appealing back. i'm not best @ writing, wanted have proofread/reviewed before sent out. don't last part, can't think of how fix it. thanks!

dear (office of financial aid),

writing letter in hopes of re-instating federal perkins loan. misunderstanding of requirements year lead me missing deadline, in turn lead cancelation of loan.

approved financial aid last year, , completed requirements without issue. year approved again. completed them again, told of requirements required completed once , 4 years. believing complete, did not peruse further. on october 4th, came financial aid office in student services building inquire balance on account. told me loan had been approved, still needed fill out promissory note in order dispense funds. however, learned deadline october 1st , had passed. told submit letter appeal re-instate loan.

out-of-state student limited family support. financial aid receive appreciated. if still possible have federal perkins loan re-instated, appreciated.
