Why are Toyotas being Recalled?

when first started happening toyota tried blame company made gas pedals them company said made pedals according design given toyota. toyota designed faulty pedals , admitted design wasn't tested enough , recall started.

i've heard 2 stories leading same answer.

first, pedal sticks due friction on own , not returning natural position. fair enough.

second, floor mat issue getting in way.

what's going on here?
