Insurance companies messed up!?!?

on 3 years ago, pick-up truck crushed entire back-side of van.
of course, wasn't fault , took care of situation calling truck driver's insurance company.
however, when going through auto insurance profile, saw weird, driving history stating at-fault accident 3 years ago.
it's weird because claimed accident not on insurance company, truck driver's.
can fix this? paying more premium because of mistake? thank you
